IUI

Intrauterine insemination (IUI) — a type of artificial insemination is a procedure for treating infertility.

Sperm that have been washed and concentrated are placed directly in your uterus around the time your ovary releases one or more eggs to be fertilized.

The hoped-for outcome of intrauterine insemination is for the sperm to swim into the fallopian tube and fertilize a waiting egg, resulting in a normal pregnancy. Depending on the reasons for infertility, IUI can be coordinated with your normal cycle or with fertility medications.

Why it’s done

A couple’s ability to become pregnant depends on many different factors. Intrauterine insemination is used most often in couples who have:

    • Donor sperm. For women who need to use donor sperm to get pregnant, IUI is most commonly used to achieve pregnancy. Frozen donor sperm specimens are obtained from certified labs and thawed before the IUI procedure.
    • Unexplained infertility. IUI is often performed as a first treatment for unexplained infertility along with ovulation-inducing medications.
    • Endometriosis-related infertility. For infertility related to endometriosis, using medications to obtain a good-quality egg along with performing IUI is often the first treatment approach.
    • Mild male factor infertility (subfertility). Your partner’s semen analysis, one of the first steps in the medical assessment of infertility, may show below-average sperm concentration, weak movement (motility) of sperm, or abnormalities in sperm size and shape (morphology). IUI can overcome some of these problems because preparing sperm for the procedure helps separate highly motile, normal sperm from those of lower quality.

IVF

During IVF, mature eggs are collected (retrieved) from ovaries and fertilized by sperm in a lab. Then the fertilized egg (embryo) or eggs (embryos) are transferred to a uterus. One full cycle of IVF takes about three weeks. Sometimes these steps are split into different parts and the process can take longer.

IVF is the most effective form of assisted reproductive technology. The procedure can be done using your own eggs and your partner’s sperm. Or IVF may involve eggs, sperm or embryos from a known or anonymous donor.

When should you consider IVF?

IVF is usually considered if you (or your partner) have certain health conditions. These include:

    • Ovulation Disorder: This is when ovulation is infrequent or absent in women, resulting in fewer eggs for fertilization.
    • Fallopian tube blockage or damage: A damaged or blocked fallopian tube makes it difficult for an egg to be fertilized or an embryo to travel to the uterus.
    • Endometriosis: This is an often painful disorder in which tissue similar to the tissue that normally lines the inside of the uterus grows outside the uterus.
    • Genetic Disorder: When you or your partner is at risk of passing on a genetic disorder to your child, IVF is considered. Uterine Fibroids: Fibroids are abnormal growths that develop in or on a woman’s uterus. Sometimes these tumours become quite large and may lead to infertility. Impaired Sperm Production: This occurs when men have little or no viable sperm in the semen.

ICSI

In intracytoplasmic sperm injection (ICSI), a single healthy sperm is injected directly into each mature egg. ICSI is often used when semen quality or number is a problem or if fertilization attempts during prior in vitro fertilization cycles failed.

Intracytoplasmic Sperm Injection (ICSI) is an assisted reproductive technique utilized to treat moderate to severe form of Male Factor infertility characterized with oligozoospermia (i.e. low sperm count) and asthenozoospermia (i.e. poor motility). It is the injection of a single sperm cell directly into an oocyte (egg) to create an embryo. ICSI is a modified form of Invitro Fertilization as fertilization takes place outside the woman body, the only different being that in conventional IVF sperms and oocytes (egg) are cultured together in an effort to achieve fertilization.

The advent of intracytoplasmic sperm injection has revolutionized the treatment of male infertility, as individuals with moderate to severe form of oligoasthenozoospermia (i.e. low sperm count and poor motility combined) can give birth to genetically related children.

ICSI procedure is performed in the embryology by an embryologist using a special kind of micromanipulation equipment with aid of inverted microscope, holding needle which is used to stabilize the egg and injection needle used to inject the sperm into the egg.

Following egg retrieval, the retrieved eggs are denuded (i.e. removal of the cells surrounding the egg) and the sperm cells are washed and screened, the selected sperm is immobilized and loaded into the injection needle. The egg to be inseminated is stabilized using the holding needle and gently the shell (zona pellucida) of egg is pierced with the injection needle to reach the egg and the sperm is gradually released into the egg’s ooplasm. The inseminated egg is then returned back to the incubator for culturing and hopefully may result to pregnancy.

Who Qualifies For This Service?

Intracytoplasmic sperm injection is indicated in patients with the following;

  • Moderate to severe form oligoasthenozoospermia (i.e. low sperm count and poor sperm motility)
  • Patients who had in-vitro-fertilization before, but had a low fertilization rate
  • Couples with obstructive azoospermia requiring the use testicular sperm extraction (TESE) or microscopic epididymal sperm aspiration (MESA).
